APPLICANT ELIGIBILITY REQUIREMENTS AND APPLICATION INSTRUCTIONS
Dear Student,
The UPLIFT Community Foundation’s scholarships are awarded for full-time and part-time programs of study and may cover academic or vocational fields of study. Applicants are encouraged to identify high-growth career fields, which will enable them to enter, re-enter, or advance in our highly technical society. The foundation’s Scholarship/Grants program is designed to provide financial assistance to students so that they may achieve their educational and professional goals. The foundation offers two (2) types of services under its scholarship program. The two types of programs are: Direct Scholarships and a Scholarship Referral Service through our Referral Services programs.
When submitting your Scholarship Application for a Direct Scholarship, please follow the foundations checklist for eligibility and informational requirements below.
1. Eligibility Requirements:
· Must be a senior in high school, a high school graduate, pass a GED test, or currently attend an accredited program of study at a United States nonprofit, tax exempt educational institution. · Must maintain a grade point average (GPA) OF 2.5 (c+) or above. · Must be in critical need of financial assistance. · Must have demonstrated community service involvement. · Must provide three (3) letters of recommendations: 1. One from a high school or college teacher. (If neither one applies, substitute one from a member of the community. 2. One from a personal reference. (Other than a relative) 3. One from a community service program supervisor. . 2. Instructions: · When submitting your Scholarship Application, type or print clearly · Submit your application between February 1-April 15 of the current calendar year. · Complete the Scholarship Application form properly. Your application will be considered incomplete unless all questions are answered and all requirements are met. · Draft a two-to-three paragraph cover letter stating why you are pursuing your career and why you should receive a scholarship. · Obtain three letters of recommendations. · Submit a copy of your current transcript. · Supply copies of PSAT or SAT test results. (If you have not been accepted in the school you plan to attend) · Include a copy of your resume if you have had work experience. (Paid or Volunteer. Service) · Supply verification of your current income, (if presently employed). · Provide a budget for the current school year. (See Pg. 2 of Scholarship Application) · Supply verification of your parent current income if they claim you as a dependent. · Submit all required application documents together and post mark by the April 15th deadline. · Individual documents mailed separate will be returned.
